I am a new patient, what do I need to bring with me for the appointment?
- Valid picture ID with current information
- Health insurance Card
- Payment
- List of medications
- Referral if insurance plan requires one
- Any radiology reports (Ultrasound, CT scan, Hida scan, MRI)
- Any test results and physician notes pertaining to the present problem you will be evaluated for.
For our new patients being evaluated for a breast condition:
Please bring any Ultrasound, Mammogram, and MRI films along with the reports

Will you request a referral from my primary care physician?
No. It is the patient’s responsibility to obtain any referrals and/or authorizations prior to your scheduled appointment.

Will surgery be performed on the same day as my visit?
Most surgeries are performed as an outpatient service at Florida Hospital or the Ambulatory Center. There are times when the physician will perform very minor procedures such as an incision and drainage of an abscess in the office under local anesthesia.

How will I know if the doctor will perform a surgery in the office?
When you are evaluated at the time of your visit the doctor will discuss with you any options for surgery in office.

Should I prepare for surgery before my visit at the office?
No preparation is needed. If you will have a minor procedure performed in the office it will be under local anesthesia (to numb the surgical site) and you will be able to drive home.

Are all major procedures performed laparoscopic?
No. There are some procedures that can not be performed laparoscopic. There are also some patients who do not qualify for laparoscopic procedures.

How much notice should I give for cancellations?
24 hour notice is required. Failure to give proper notice for a cancellation may result in a charge of $25.00

When may I return to work after surgery?
Several factors that determine when a patient may return to work include the type of surgery that was performed and the nature of the patient’s employment (e.g. whether the work requires heavy lifting or prolonged periods of standing or walking). Your surgeon will make the determination when it is safe to return to work.
